This will allow you the option of reviewing plan information at your own pace and in the comfort of your own home if you so desire. Our goal is to provide you with all the planning tools necessary to make informed decisions regarding your benefit coverage.
Your total compensation package includes many benefits that are largely paid for by MHMRA of Harris County on your behalf. We also offer access to optional, supplemental benefits that you can elect as part of your package if you think they will be beneficial. The premium for the voluntary plans will be paid for by you through payroll deduction.

MHMRA of Harris County provides a comprehensive benefit package designed to meet the needs of our employees and their families. We offer many benefits, including medical and dental coverage, a retirement plan, basic life insurance with AD&D and voluntary supplemental life and AD&D coverage available for you and your dependents as well. Also provided are long term disability coverage, a vision plan, an employee assistance plan, flexible spending accounts and Colonial individual policies.

Eligibility...
You are eligibile to enroll in benefits on the first of the month following 30-days of full-time employment. Full-time employment is defined as a minimum 30-hour work week.
The benefits and rates shown in this site are for full time employees. Part time employees have access to benefits as well, though the rates and/or employee premium contributions will differ. Please contact HR for more information.
Dependent Verification
In order to enroll your legal dependents, you must provide proof of their relationship to you. Proof of legal dependent status includes a birth certificate and a marriage license. Proof of legal dependent status must be furnished within 30 days of date of hire. Failure to provide proof of legal dependents will result in denial of coverage for persons not validated in this manner.
COBRA notification
If your hours are reduced below the 30-hour minimum threshold, you will be offered an opportunity to maintain your enrollment in the core programs at you own expense. Please download your initial COBRA notification to view your rights under COBRA.
Reinstatement
If you return to full-time status within 30 days of having been terminated from employment, or due to a reduction in hours, your benefits coverage may be fully reinstated, though the employee will be required to pay for one month of premium.
Note that benefits could be reinstated for other purposes, such as return to duty after being called to serve in the military (USERRA) or family leave as defined by the Family Medical Leave Act (FMLA). Please see the Summary Plan Document for each plan to see how USERRA and FMLA apply.

We offer two medical plans which are administered by CIGNA, one of the largest health plan administrators in the nation. The first plan is an HMO and the other is an Open Access HMO. With the HMO you will need to choose a primary care physician and you will also need to work strictly within the provider network for non-emergency care. The Open Access HMO is much like a PPO in that you have out of network coverage for non-emergency care and you do not need to choose a primary care physician.

HMO Mental Health Substance Abuse Amendment
CIGNA provides customer focused health care, dental and related benefits offered to individuals and through employers. Key product lines include: Health care and dental products and services as well as group disability, life and accident insurance.

MHMRA of Harris County provides voluntary employee paid vision insurance through EyeMed. This plan provides benefits for eye exams, eyeglasses, contact lenses, and contact lens fittings and evaluations. To locate a provider near you please see the EyeMed link on the Tools page. MHMRA of Harris County provides Long Term Disability (LTD) Coverage to all full time eligible employees. LTD is an income replacement program that protects you and your family in the event that you become disabled and are unable to perform the material and substantial duties of your job.
If an employee becomes disabled, the plan pays a benefit of up to 60% of covered monthly income for a maximum 5 year period. The minimum monthly benefit is the greater of $50 or 10% of gross disability payment. The maximum monthly benefit is $7,500. The plan includes a rehabilitation benefit which could pay for the costs associated with rehabilitation.
Long Term Disability income insurance helps you pay living expenses while you are unable to work. While it is not designed to replace your income, it can help by providing cash or mortgage or rent payments, groceries, utilities and other necessary expenses.

If you suffer a covered disability while insured by this plan, you will receive monetary benefits designed to help you maintain your normal lifestyle.
This program covers disabling injuries or sickness while off the job which last beyond the waiting period.

This Short Term Disability Plan (STD) pays a benefit of up to 60% of your weekly covered earnings to a maximum of $2000 per week until the end of the 12 week benefit period for accident or the end of the 11 week benefit period for sickness, or until you no longer qualify for benefits.

MHMRA of Harris County offers a competitive Group Life and Accidental Death and Dismemberment (AD&D) insurance program, including both employer paid plans and employee paid programs from which you can choose to voluntarily supplement your life and AD&D insurance.
Basic Group Life and AD&D Insurance
Your Basic Life Insurance benefit is two times your salary to a maximum of $350,000. The Basic AD&D benefit is three times your salary to a maximum of $350,000. Basic Life and AD&D coverage is paid for by MHMRA.
Optional Voluntary Employee Group Life and AD&D Coverage
As an eligible employee, you may also purchase optional Group Term Life Insurance coverage for yourself in the amounts of 1x, 2x, 3x or 4x your annual salary. Maximum benefit is the lesser of 4x your annual salary or $500,000. Amounts of your salary are guarantee issue but only within your new hire election period. The guarantee issue amount is the lesser of 2x your annual salary or $200,000. If you choose to enroll after your her hire election period, (first of the month following 30 days of continuous employment), Evidence of Insurability will be required for any amount of voluntary life insurance. Additionally, amounts exceeding guarantee issue will always require that you complete and submit an Evidence of Insurability Form to the carrier to be approved for coverage. Employees pay the full cost of optional coverage based on the amount of coverage elected and their age. Optional Voluntary AD&D coverage is also available. Dependent Coverage
Employees who have elected and been approved, if applicable, for optional life insurance coverage for themselves are eligible to elect optional coverage for their spouse and children as well. You may purchase 100% of your annual salary up to $50,000 of Voluntary Group Term Life Insurance for your spouse; the amount you pay for the coverage is based on your own age. You may purchase 20% of your annual salary up to $10,000 for your children. Guarantee issue amounts are $30,000 for your spouse and $10,000 for children. Voluntary AD&D coverage is also available for your spouse and children. For more information on voluntary dependent coverage, please see the brochure in the Forms Section of the website.
Note: There is NO annual open enrollment for employee and dependent Voluntary Life and AD&D coverage! This coverage is available without Evidence of Insurability ONLY when you are first eligible. Please submit enrollment paperwork during your new hire enrollment period.
Conversion and Portability
In the event that you separate from service at MHMRA, you will have the option of converting your employee Group Basic Life Insurance amount that was in place at the time of your termination to an individual life policy. You also have portability rights associated with your Voluntary Group Life Coverage. Please see the Certificate of Coverage for more information on conversion and portability rights, including terms and conditions.
Beneficiary
It is important that you name a beneficiary to receive benefits from the life insurance plan in the event of your death. It is important to keep your beneficiary designation up to date and to let your beneficiary know about your MHMRA life insurance coverage.

Assistance is available to employees experiencing problems that may affect their attendance or work performance. The EAP plan offers phone and individual counseling on personal, legal, financial, or substance abuse problems.

Use the link button, below, to login into the MHMRA secure enrollment site. Note that you must scroll to the bottom of the opening page, where you will be required to register with a unique username and password. Members MUST enroll for benefits online within 30 days of their change date or they will not be eligible until the next Open Enrollment period. Members MUST ALSO make any qualified changes to benefits during the year online within 30 days of the qualified Life Status Change Event.
